Can Hayat operates under the PRIZMATION name. For personal data enquiries and requests, contact can@prizmation.com.
Information and purpose
The web form records your name, selected contact channel, email address or WhatsApp number for that channel, your description of the process or need, and the notice checkbox status. Form language/source, submission time, time zone and platform technical records, such as an IP address, may also be recorded. We use this information to assess your enquiry, respond through your selected channel and follow up on your request. Do not include patient information, health data or unnecessary information about other people.
Form and service providers
HighLevel/LeadConnector provides the form and stores submitted information in its enquiry/contact records. Netlify hosts the website. HighLevel and technical providers may process data outside Türkiye. Loading the widget may also establish third-party connections to Google Fonts and Meta resources, in addition to HighLevel resources.
Collection and checkbox meaning
Data is collected electronically through the web form using automated means. Checking the box only acknowledges that you have read this notice and received the information; it is not marketing permission or explicit consent to overseas transfers. Confirmation of the applicable processing conditions and overseas transfer mechanism is ongoing; this notice does not replace them.
Your rights and requests
Under Article 11 of Turkish Law No. 6698, you may request information about processing, its purposes and recipients; correction of inaccurate or incomplete data; deletion or destruction where the conditions apply; and notification of those actions to recipients. You may object to adverse outcomes from exclusively automated analysis and seek compensation for damage from unlawful processing.
